It does NOT apply to:
Protected Health Information (PHI) we collect as a healthcare provider and Durable Medical Equipment (DME) supplier. PHI is governed by our HIPAA Notice of Privacy Practices, available at [link to HIPAA NPP] or by request.
Information collected offline, through other websites, mobile apps, or third-party services.
Employee or job applicant information.
If you are a patient, your health information is protected under HIPAA. This Site is not intended to collect PHI. Do not submit diagnosis, prescription, or detailed medical info through contact forms. Call us at 888-688-4965 instead.

A. Information You Provide Voluntarily
Contact Info: Name, address, email, phone number submitted via forms.
Service Inquiries: Insurance provider, physician name, equipment type requested.
Account Info: Username/password if you create a patient portal account.
Communications: Info in emails, chats, or SMS replies to us.
B. Information Collected Automatically
Device & Usage Data: IP address, browser type, pages viewed, date/time, referring URL.
Cookies & Similar Tech: We use essential cookies for site function and analytics cookies (Google Analytics) to understand traffic. You can disable cookies in browser settings.
C. We Do NOT Knowingly Collect: Data from children under 13. Do not use this Site if under 13.
HOW WE USE YOUR INFORMATION
We use information to:
Provide Services: Respond to inquiries, verify insurance, schedule deliveries, coordinate DME setup.
Communicate: Call, email, or text you about appointments, delivery ETAs, equipment instructions, or service updates if you consent.

Legal & Compliance: Comply with Medicare, Medicaid, CA HMDR, FDA, and other laws; prevent fraud; respond to subpoenas.

SMS/TEXT MESSAGING TERMS
If you provide your mobile number and consent to receive texts:
Purpose: We send transactional messages: appointment reminders, delivery notifications, equipment setup help, service alerts.
Frequency: Varies by service need. Typically 2-5 msgs per delivery.
Message & Data Rates: May apply per your carrier plan.
Opt-In: You may Opt-In verbally.
Opt-Out: Reply STOP to any message to cancel. Reply HELP for help.
Consent Not Required: SMS consent is not a condition of purchase or receiving DME services.
No Sharing: We do not share your mobile phone number, SMS consent, or SMS content with any third parties, affiliates, or partners for their marketing or promotional purposes. We may share with vendors who assist in sending messages under confidentiality agreements.
This SMS clause is required by mobile carriers for 10DLC compliance.
HOW WE SHARE YOUR INFORMATION
We do NOT sell or share your personal information for monetary consideration or cross-context behavioral advertising.
We disclose information only as follows:
Service Providers: Billing, IT, hosting, SMS vendors under written contracts requiring confidentiality + HIPAA compliance where applicable.
Healthcare Operations: With physicians, insurers, Medicare/Medicaid as permitted by HIPAA for treatment, payment, or healthcare operations.
Legal: To comply with law, subpoena, or to protect rights, safety, or property.

Delete: Request deletion, subject to legal exceptions like Medicare 7-year retention.
Correct: Request correction of inaccurate info.
Opt-Out of Sale/Sharing: We do not sell or share data. You may still submit a request.
Limit Use of Sensitive Info: We only use sensitive info for permitted business purposes.
Non-Discrimination: We will not discriminate for exercising rights.

In the last 12 months, we collected: Identifiers, Commercial Info, Internet Activity, Geolocation. We disclosed for business purposes to IT, billing, and SMS vendors. We did not sell/share.
DATA SECURITY & RETENTION
We use reasonable administrative, technical, and physical safeguards per HIPAA Security Rule and NIST standards. However, no internet transmission is 100% secure.
We retain website data only as long as needed for purposes described or as required by law. Medicare records: 7 years per 42 CFR §424.57.

INTERNATIONAL USERS
Site is operated in the U.S. If you access from outside U.S., you consent to transfer/processing in U.S.
